Company Information – CTD

1. Click the Company Type drop down box and select CTD.

2. Type your CTD’s 8-digit ARC# in the ARC/ACN Number textbox.

3. Type your CTD’s name in the Company Name textbox.

4. Type your CTD’s address in the Address textboxes.

5. Type your CTD’s city in the City textbox.

6. Type your CTD’s zip code/postal code in the Zip/Postal Code textbox.

7. Click the State drop down box and select your CTD’s state.  Note: Depending upon country selected, this field maybe labeled State, State/Province or Province.  If labeled State/Province, type the name of your State/Province in the State/Province textbox.  If labeled Province (only), click the drop down box and select a Province.

8. Click the Country drop down box and select your CTD’s country.
