Company Information – Agency

1. Click the Company Type drop down box and select Agency.

2. Type your agency’s 8-digit ARC# in the ARC/ACN Number textbox.

3. Type your agency’s name in the Company Name textbox.

4. Type your agency’s address in the Address textboxes.

5. Type your agency’s city in the City textbox.

6. Type your agency’s zip code/postal code in the Zip/Postal Code textbox.

7. Click the State drop down box and select your agency’s state.  Note:  Depending upon country selected, this field maybe labeled State, State/Province or Province.  If labeled State/Province, type the name of your State/Province in the State/Province textbox.  If labeled Province (only), click the drop down box and select a Province.

8. Click the Country drop down box and select your agency’s country.
